Case management is a service designed to assist People Living With HIV/AIDS (PLWH) to achieve the best health outcomes. Case managers work with individuals one-on-one to ensure PLWH have access to medical care and other support services.
Having a case manager on your care team gives you one more person to help you manage your HIV care, work with your medical providers, and help you access services you need. Case managers are located across the state of Iowa and are here to help!
